Designing for the top fold

There is a lot of talk about the "top fold" or the "first screen pull" and how far uses travel vertically down the page to read content on a page. Indeed you see advice from some leading authorities about "eyeball tracking" and keeping users engaged, the do's and dont's can lead us to think that unless it's above the fold or the first screen pull it's going to get missed. But is this all what we think it is? Are we engaging our users at the right point of the webpage?
